In accordance with Article 13 of the Law, applications regarding these rights must be submitted in writing through the following methods specified in the relevant legislation to \u00d6zel ENA Laboratuvar G\u00f6r\u00fcnt\u00fcleme Medikal T\u0131bbi Cihazlar Pazarlama Ticaret A.\u015e. (\"Institution\"), the data controller located at Barbaros Mah. Bu\u011fday Sok. No:2\/3 \u00c7ankaya Ankara. <strong>In this context, if the application is to be made in writing, the form below must be completed. The relevant form can be submitted to the Institution via:<\/strong><\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Personal application by the applicant,<\/li>\r\n\r\n\r\n\r\n<li>Through a notary public,<\/li>\r\n\r\n\r\n\r\n<li>Via registered mail with return receipt,<\/li>\r\n\r\n\r\n\r\n<li>By signing with an electronic signature compliant with the Electronic Signature Law No. 5070 and sending it to the Institution's registered email address,<\/li>\r\n\r\n\r\n\r\n<li>By sending via the email address previously notified to the Institution and registered in the Institution's system,<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p>In accordance with Paragraph 2 of Article 13 of the Law, your request will be responded to as soon as possible depending on the nature of the request, and within thirty days at the latest. Additionally, during the evaluation of the relevant requests, additional information may be requested from the applicant to verify the application and ensure personal data security.
